Birth Parent
Manitoba Post-Adoption Services for Birth Parents:
A copy of the Pre-Adoption Birth Registration and Substitute Birth Registration (for a child 18+ years of age):
The Pre-Adoption Birth Registration lists your child's birth name and birth details as well as the birth mother's name and demographic information at the time of child's birth.
The Substituted Birth Registration lists your child's full name by adoption. To comply with Legislation the adoptive parents' identifying information is removed from the Substituted Birth Registration.
NOTE: A search can only be performed for individuals named in the Department's records. A search is not performed for any person under 18 years of age or who has filed a Vetoes, Contact Preferences, or past refusals.
Birth Fathers Disclaimer: Manitoba Vital Statistics Agency only lists fathers on the birth document if the parents were married at the time of the birth or they signed a joint request to have the father listed on the document. Birth fathers not listed on the pre-adoption birth registration will not be entitled to copies of the pre-adoption birth registration or the substituted birth registration. Manitoba Post-Adoption Services will not know if the birth father is named on this document until an application is processed.
Eligibility:
A birth parent of a child born in Manitoba (and is now 18+ years of age) and their adoption granted in Manitoba or another jurisdiction.

Adoptive Family Profile:
An Adoptive Family Profile provides you with available information from the records on the child's adoptive parents including information about:

- your child's development and wellbeing prior to the adoption being finalized.
An Adoptive Family profile is shared in a non-identifying manner and will not contain names, full dates of birth or other identifying details. The amount of information available varies case by case and is dependent on the information that was gathered and recorded at the time of your child's birth and adoption planning.
Eligibility:
A birth parent of a child who had their adoption granted in Manitoba.
A birth parent of a child who had their adoption granted outside of Manitoba, must apply for services through the jurisdiction where the adoption was granted. If you are unsure about where the adoption occured, Manitoba Post-Adoption Services may be able to provide you with information on the jurisdiction where the adoption was finalized.
