Background Checks

The applicant(s) and residents in the home must obtain the following checks:

  • Criminal Record Check with a Vulnerable Sector Search that must include a transcript/print out of any criminal convictions and/or outstanding charges if applicable. These checks are done for individuals 18 years and older and must be completed by local Police Services or the RCMP based on the person’s location address. The results must be submitted to the Early Learning and Child Care Program.

  • Criminal History Self Disclosure Statement and Information Release Consent available online to be printed out, completed and submitted to the Early Learning and Child Care Program for individuals 18 years and older.

  • Child Abuse Registry Check and a Prior Contact Check for individuals 12 years and older. You will need to obtain both of these forms from the Early Learning and Child Care Program and submit to the Child Abuse Registry.  

    If I am submitting these forms by mail, I must send two photocopied pieces of valid, government-issued identification that are signed and dated by a Commissioner for Oaths, a Notary Public, or a professional who witnessed my signature and saw the original identification. My witness must also provide contact information, including their occupation or designation, place of employment, address and a daytime telephone number where they can be reached.

    For the purposes of identification, a professional is considered to be one of the following:
    • Dentist/Medical Doctor/Chiropractor/Optometrist/Psychologist
    • Lawyer
    • Minister of Religion
    • Pharmacist
    • Principal or teacher at a primary or secondary school
    • Judge/Magistrate/Police Office/RCMP
    • Justice of the Peace
    • Postmaster
    • Professional Accountant who has a designation
    • Signing Officer or Manager at a bank, credit union, trust company, or other financial institution
    • Senior Administrator, teacher, professor at a community college or university
    • Veterinarian
    • Social Worker
    • Chief of First Nations Band
    • Funeral Director
    • Nurse Practitioner/Registered Nurse
    • Member of Parliament
    • Member of the Provincial Legislature
    • Municipal Office
    • Official of a federal or provincial government department, or one of its agencies
    • Official of an embassy or consulate
    • Professional Engineer
