To register for a Manitoba Health Card you need to provide copies of documents that prove your citizenship or immigration status in Canada and your residency in Manitoba. This tool will help you determine what documents you are required to provide.

You must provide a copy of a document proving your legal status in Canada. Please select an option from this list and then click Next.
Select one of the following:
Your Canadian passport must be currently valid. Name recorded on passport must be full legal name.
An official birth certificate issued by the vital statistics agency of a Canadian province or territory. These documents are not eligible: divisional or district birth certificates; commemorative birth certificates, Baptismal certificates, and notices of live birth issued in Manitoba.
A plastic card issued prior to Feb. 1, 2012.
A letter-sized certificate issued after Jan. 31, 2012.
This card confirms status as a Treaty Indian under the Indian Act.

A laminated identification card that is issued by Immigration, Refugees, and Citizenship Canada confirming permanent resident status.
A Confirmation of Permanent Residence (COPR) document is issued by the Canadian government, confirming your permanent resident status and the date permanent resident status was conferred.
Your official work permit issued by Immigration, Refugees, and Citizenship Canada. It contains an issue date an expiry date that form the dates of potential eligibility for the Manitoba Health insurance plan.Accompanying spouse and/or minor children may also need proof of relation (marriage certificate/birth certificate [translated to English or French if necessary] to the eligible work permit holder if it is not stated on their permit).

Proof of Manitoba residency
You must provide proof of residence in Manitoba for 6 months (183 day) in a calendar year. Please select an option from this list then click Next.
Select one of the following:
Mortgage documents must be issued by the financial institution that holds the mortgage, and must show your name and address. A Land Titles document must show your current name and address. Letters from lawyers or realtors are not acceptable.
Lease issued by a property management company or private landlord must contain your name and address, the leasing company’s name and address, your signature and a leasing company signature. If you are subletting or added mid-term, a letter from a tenancy agency with your name and start of occupancy must accompany the original lease.
A rental agreement between family members residing in the same residence is also acceptable, providing it contains the applicant’s name, address of rental, and signatures of applicant and renter. This agreement must be signed by a Manitoban Notary Public or Manitoban Commissioner of Oath to confirm its legitimacy.
A confirmation letter from your Manitoba employer. The letter must be on company letterhead, and must contain your name, the employer’s name, type of employment (full time, part time, contract) and the duration if not permanent employment. This document must contain an issue date no more than one month (30 days) before the date of submission to our office.
This letter must state: 1) names of the applicants for Manitoba Health coverage living with them in the residence, and 2) intended length of stay at the residence beyond the writing of the letter. It must be signed by a Manitoban Notary Public or Manitoban Commissioner of Oath. This document must contain an issue date no more than one month (30 days) before the date of submission to our office.
This letter would be provided by a supporter organization, confirming intended residency in Manitoba. This option is only available to individuals receiving assistance from a known Resettlement Assistance Program or Sponsorship Agreement Holder operating in Manitoba. This document must contain an issue date no more than one month (30 days) before the date of submission to our office.
A letter from a Newcomer Service Provider confirming residency in the province signed by a Manitoban Notary Public or Manitoban Commissioner of Oath.

The bill must be from a recognized utility for ongoing utility service at a physical residential address in Manitoba. This includes natural gas, hydro, waster/water, landline telephone and cable TV. It does not include bills for repairs and maintenance or initial set-up of services. The bill must show your name and address, and all pages of the bill must be presented. Printed copies of electronic bills are accepted. Printed copies of e-bills and screenshots of e-bills if stamped by the service provider are acceptable.
This option must include your name, dwelling address covered under the policy, and must be for a residence in Manitoba that you are actively living in. Timeline restrictions apply.
The bill must include your name and address.
The licence/identification card provided must contain your current mailing address, or physical address if living rurally where you use a post office box or other mailing address not listed by Manitoba Public Insurance.
The registration papers must be valid and signed. Storage insurance registrations or papers that state "No Registration" are not accepted.
The document must be issued by an accredited educational institution in Manitoba and confirm the period of enrolment. Timeline restrictions apply. This document must contain an issue date no more than two months (60 days) before the date of submission to our office.
Full document must be provided. E-filing statements or statements from a tax filing company or private accountant are not accepted, we require the official statement issued by Revenue Canada.
